PATIENT RIGHTS

Communicate and obtain information while respecting patient rights, privacy, and confidentiality of information in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA)
Involve the patient and family and respect their role in determining the nature of care to be provided
Meet patient's and families' needs regarding communication, including interpreter services
Provide accurate information to patients and families in a timely manner

INTERDISCIPLINARY COMMUNICATION

Communicate with patients and their companions in a courteous manner using age and cultural appropriate methods.
Understand the chain of communication, who to contact, and when.
Listen actively to patient concerns and address them appropriately.
Perform and maintain accurate patient records and documentation related to interventions, plan of care, diagnoses.
Maintain patient confidentiality according to HIPAA guidelines i.e.: status updates to family members.
Implement effective solutions for communication with patients when barriers exist i.e.: physiological, cultural, language.

CUSTOMER SERVICE

Assist patients or clients into the facility
Conducts initial screening to determine the treatment level needed for each potential patient
Enter admission information into the digital system and complete all admission paperwork
Coordinates patient/referral information to appropriate intake staff and or department
Carry out scheduling of evaluations, meetings, and consultations for patients
Assist staff, patients/patients' families and healthcare staff as needed to resolve basic patient related issues
Communicates with all customers for all internal and external activities by providing accurate information via phone call,email or fax
Provide follow up calls if required to patients and other staff
Coordinates patient care, housing, medication administration and documentation with skilled nursing facilities, insurance companies, pharmacies, and hospitals
Performs patient visits as needed to address urgent needs or staffing shortages as assigned by the clinical supervisor
Treats patients with utmost respect and kindness while ensuring excellent customer service

GENERAL RESPONSIBILITIES

Answers incoming phone calls and email inquiries
Coordinates information with clinicians, core staff and other team members by utilizing a computer database system
Distribute all mails and provide facsimile transmissions and coordinate with team members and health service
Documenting patient charts while maintaining electronic and/or hard copy files as directed
Handling patient or physician complaints and forwards to appropriate administrative staff
Maintaining updated clinician’s schedules and availability according to authorizations and patient needs
Maintains a tracking system to ensure clinical records are up to date according to company policy and regulatory requirements
Operates personal computer to access agency software, e-mail, electronic calendars, and other basic office support software
Performs clerical work, such as filing, copying, or faxing
Verifies with insurance companies to inquire about insurance coverage
Works professionally and preserves harmonious working relationship with clinicians, staff, and other team members in the facility
