We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) to provide quality patient care in a Skilled Nursing Facility (SNF) setting. The LPN/LVN will be responsible for medication administration, treatments, patient education, accurate documentation, and compliance with established facility policies and regulatory standards.
Responsibilities
Provide direct care to patients, including medication administration and treatments.
Provide patients with appropriate medication education.
Complete accurate and timely documentation of patient care and services provided.
Perform computer data entry; full training will be provided.
Follow all facility Standard Operating Procedures (SOPs) and established clinical guidelines.
Maintain compliance with applicable standards and requirements, including ICF-IDD regulations, DSSLC policies and directives, and HHSC standards, rules, and directives.
Support a safe, compliant, and patient-centered care environment.
